Fax/Mail/Email us your W-9 and a copy of your Non-Profit Letter [501(c)(3)],
as issued by the IRS.
These forms can usually be obtained from your office manager or your
organization's accounting offices.

That's it! Once approved, you will be listed on Bloomin' Bucks website as a
participating organization and we will contact you again letting you know
